Lowering asymmetric dimethylarginine--a new mechanism mediating the renoprotective effects of renin-angiotensin system inhibitors in proteinuric patients?
نویسندگان
چکیده
the ACE inhibitor and the ARB lead to similar improvements of the biochemical markers as well as to enhancement of flow-mediated vasodilation. Interestingly, at baseline, the degree of proteinuria was independently related to ADMA and SDMA levels. Moreover, logistic regression analysis suggested that the 35% drop in ADMA levels in this study was related to the improvement of proteinuria. Despite all the limitations of this nonrandomized uncontrolled study, the authors point out a new and potentially important pathway by which proteinuria and renal injury could be linked. A reduction of proteinuria by blocking the renin-angiotensin system could cause a decrease in ADMA (and SDMA), potentially increasing the bioavailability of nitric oxide. This effect seems to be independent of the changes in blood pressure.

New approaches to blockade of the renin-angiotensin-aldosterone system: mineralocorticoid-receptor blockers exert antihypertensive and renoprotective effects independently of the renin-angiotensin system.
The role of angiotensin II in mediating hypertension and renal diseases is well documented, and inhibition of the renin-angiotensin-aldosterone system elicits antihypertensive and renoprotective effects. There is increasing evidence implicating aldosterone, in addition to angiotensin II, in the pathogenesis of hypertension and renal diseases. Beneficial effects of mineralocorticoid receptor (MR...

Background and aims: Diabetic nephropathy (DN) is the most common cause of end-stage renal failure which could increase the risk of cardiovascular disease and morbidity and mortality in patients. The aim of this study was to investigate new modalities for treatment of diabetic nephropathy. Methods:This study was a mini-review research to investigate drugs that are used for DN treatment. متن کاملRenin-angiotensin system is involved in the mechanism of increased serum asymmetric dimethylarginine in essential hypertension.
Endothelium-dependent/nitric oxide (NO)-mediated vasodilation is impaired in hypertensive individuals. Asymmetric dimethylarginine (ADMA), an endogenous inhibitor of NO synthase, is synthesized by many types of cells including vascular endothelial cells. The serum level of ADMA is elevated in patients with essential hypertension, but the mechanism for this increase is unknown. Therefore, the pr...
متن کاملRenin-Angiotensin System Inhibitors Reduce Serum Asymmetric Dimethylarginine Levels and Oxidative Stress in Normotensive Patients with Chronic Kidney Disease
BACKGROUND/AIMS The purpose of our study was to elucidate the relationship between asymmetric dimethylarginine (ADMA) and intrarenal lesions and to determine the effect of renin-angiotensin system inhibitors (RAS-Is) on serum ADMA levels, nitric oxide (NO) synthesis and oxidative stress in normotensive patients with chronic kidney disease (CKD). METHODS This study included 23 normotensive pat...

Despite the huge amount of studies looking for candidate genes, the ACE gene remains the unique, well-characterized locus clearly associated with pathogenesis and progression of chronic kidney disease, and with response to treatment with drugs that directly interfere with the renin angiotensin system (RAS), such as angiotensin converting enzyme (ACE) inhibitors and angiotensin II receptor antag...
